Most patients complete their new patient forms digitally before their first visit, which makes check-in faster and gives us a chance to review your information ahead of time. Paper forms are available at the office for any form you would prefer to fill out in person.

What we need

What the forms cover

The new patient packet asks for information we need to provide safe, well-coordinated care:

  • Personal information — name, contact info, emergency contact
  • Medical history — current health conditions, medications, allergies
  • Dental history — past treatment, current concerns, what brought you in
  • Insurance information — if you have dental insurance and want to use it
  • Patient consent — standard consent forms required by law

The complete packet typically takes 10 to 15 minutes to fill out. Doing it before your appointment means you get to relax when you arrive instead of filling out paperwork.

Paper forms.

If you prefer to fill out paper forms, we have all of our forms available at the office in paper format. Simply arrive a few minutes early on the day of your appointment and we will provide everything you need.

Either approach works equally well. We have many patients who prefer paper, and that is exactly fine. The goal is for you to feel comfortable and prepared, however you prefer to handle the paperwork.

What to bring on your first visit

A few simple items make the appointment go smoothly:

  • Photo ID
  • Dental insurance card (if you have insurance and want to use it)
  • Any prescription medications you currently take — or a list of them
  • A list of any specific questions or concerns you would like to discuss
  • Names and contact info of any other doctors involved in your care that may be relevant to your dental treatment

If you forget anything, do not worry — we can work around it. The most important thing is that you show up.

On the day

What you will receive at the visit

When you arrive, we will:

  • Welcome you (most likely by name, if you have given us a heads-up that you are coming)
  • Confirm we have all your forms and insurance information
  • Walk you back to the operatory
  • Begin your comprehensive exam

You will not be sitting in the waiting room filling out a stack of forms when your appointment time arrives — that is the whole point of getting the forms done in advance.

Your information is safe with us

The patient platform we use is HIPAA-compliant and securely encrypted. Your medical and personal information is protected throughout the digital process. Inside our office, only members of our team who are involved in your care have access to your records.

Common questions about forms

What if I cannot get the digital forms to work?

Call us at (352) 293-2800 and we can help. If digital just is not for you, paper forms are equally welcome.

I started the digital forms but did not finish. Can I come back to them later?

Yes. The patient platform saves your progress and you can return to complete the rest at your convenience.

Do I need to redo forms every visit?

No. After your initial new patient packet, we just verify and update information at periodic intervals (typically annually). You will not have to fill everything out again at every visit.

I am uncomfortable sharing certain medical information. Do I have to?

We ask for the information that is relevant to your safe dental care. If something gives you concern, talk to us — we are happy to explain why we ask for what we ask for.
